  1. Prepare the chocolate
    In a medium bowl, combine the cocoa powder and chopped dark chocolate. Set aside while you heat the milk.
  2. Heat the milk
    Pour the milk into a medium saucepan and heat over medium-low heat until it begins to steam and small bubbles form around the edges. Do not allow it to boil.
  3. Make the chocolate base
    Ladle about 1 cup of the hot milk into the bowl with the cocoa powder and chocolate. Whisk until smooth and the chocolate is completely melted.
  4. Combine and flavor
    Pour the chocolate mixture back into the saucepan with the remaining milk. Add sugar, vanilla extract, salt, and if using, the cinnamon and cayenne pepper. Whisk constantly over medium-low heat until the sugar is dissolved and the mixture is hot, about 2-3 minutes.
  5. Whisk to perfection
    For an extra frothy hot chocolate, use an immersion blender or transfer to a blender (being careful with the hot liquid) and blend for 10-15 seconds until frothy.
  6. Serve and garnish
    Pour the hot chocolate into mugs and top with mini marshmallows or whipped cream. For an extra touch, dust with a little cocoa powder or grated chocolate. Serve immediately while hot.
