Prepare the burger mixtureIn a large bowl, combine the ground beef, diced onion, minced garlic, Worcestershire sauce, salt, and pepper. Mix gently with your hands until just combined, being careful not to overwork the meat.

Form the pattiesDivide the mixture into 4 equal portions and shape into patties about 1/2 inch thick and slightly wider than your burger buns, as they will shrink during cooking. Press a slight dimple in the center of each patty with your thumb to prevent them from puffing up.

Preheat and cookPreheat a grill or skillet to medium-high heat. Cook the patties for 3-4 minutes on each side for medium doneness. If adding cheese, place a slice on each patty during the last minute of cooking to melt.

Toast the bunsWhile the burgers are cooking, lightly toast the burger buns on the grill or in a toaster until golden brown.

Assemble the burgersSpread mayonnaise on the bottom half of each bun. Add a lettuce leaf, followed by a burger patty (with melted cheese if using). Top with tomato slices, red onion rings, and a squeeze of ketchup. Place the top bun on and serve immediately.
